​Required Documents:
1. Proof of Citizenship
Born in the United States:
Provide an Original Birth Certificate that meets the following:
  • Must state "Birth Certificate" not "Birth Registration" or other title
  • Issued by the city, county, or state of birth
  • Lists applicant’s full name, date of birth, and place of birth
  • Lists Both Parent(s)’ Full Names
  • Has the signature of the city, county, or state registrar
  • Has the date filed with registrar's office (must be within one year of birth)
  • Has the raised seal or stamp of the city, county, or state which issued it
Born Outside the United States:
  • Consular Report of Birth Abroad or Certification of Birth
  • Certificate of Naturalization 
  • Certificate of Citizenship
** Please note ** 
- If your Birth Certificate does not have both parents names listed, or states that it is a Birth Registration, you will need to obtain a certified copy that lists your parents names before your application is submitted.
  • Contact the Office of Vital Records in the County you were born in to obtain this document.

​2. Proof of Identity (Applicants over 18)
Acceptable forms of Photo ID:
  • In-state, fully-valid driver's license or enhanced driver's license with photo
  • Valid or expired, undamaged U.S. passport book or passport card 
  • Certificate of Naturalization 
  • Certificate of Citizenship 
  • Government employee ID (city, county, state, or federal)
  • U.S. military or military dependent ID
  • Current (valid) foreign passport
  • Matricula Consular (Mexican Consular ID)
  • U.S. Permanent Resident Card (Green Card) 
  • Trusted Traveler IDs (including valid Global Entry, FAST, SENTRI, and NEXUS cards)
  • Enhanced Tribal Cards and Native American tribal IDs
Applying with an Out of State ID:

If you apply with an out-of-state ID (Non-NYS), please present an extra ID. It should show as much of the following information as possible:

Your photo
Full name
Date of birth, and
Document issuance date

​3. Additional Requirements by Age & Application Type:
Applicants who never had a passport before:

New Applicants Over 18 yrs old:
​
  • Original or Certified Birth Certificate with a raised seal and both parents names listed. 
  • Valid photo ID (driver's license or state ID card)
  • Proof of name change if different from photo ID (original marriage certificate, etc.)
 
New Applicants Ages 16 & 17:
​
  • Original or Certified Birth Certificate with a raised seal and both parents names listed.
  • Valid photo ID (Learner's Permit, Driver's License or student ID accepted)
  • ​One parent consent will be required with a valid photo ID (driver's license or state ID card)

New Applicants Ages 15 & under:
​
  • Original or Certified Birth Certificate from the Department of Health with a raised seal and both parents names listed.
  • Both parents or legal guardians must be present with a valid photo ID (driver's license or state ID card).
Applicants Under 18, with existing Minor Passport:

Applicants Ages 16 & 17:

  • Photocopy of Birth Certificate
  • Minor passport book 
  • Valid photo ID (Learner's Permit, Driver's License or student ID accepted)
  • ​One parent consent will be required with a valid photo ID (driver's license or state ID card)

Applicants Ages 15 & under:
​
  • Photocopy of Birth Certificate
  • Minor passport book 
  • Both parents or legal guardians must be present with a valid photo ID (driver's license or state ID card).
Passport Costs:

​Passport Books - New Applicants

For persons ages 16 and older:
- Passport Book: $130
- Photo: $14
- Processing Fee: $35 (includes Priority Mailing)


For persons under 16 years old:
- Passport Book: $100
- Photo: $14
- Processing Fee: $35 (includes Priority Mailing)

​Passport Cards - New Applicants​

For persons ages 16 and older:
- Passport Card: $30

For persons under 16 years old:
- Passport Card: $15

* If applying for a book and card in the same application, fees for the photo and processing are only paid once. 

**If applying for Only a Passport Card, please note the following fees will apply to the application:

- Photo: $14
- Processing Fee: $35 (includes Priority Mailing)
Passport fee payment methods:
  • ​Passport Book / Card fees for the US Dept. of State must be paid either by personal check or money order ONLY.
  • Processing fees including photos and postage can be paid by personal check, cash or credit card.

Regular Processing:
4 - 6 Weeks

Expedited Processing: Additional Fee
2 - 3 Weeks

​Expedited Processing:

To obtain your passport sooner, there is an option to expedite the process (2 - 3 weeks turn-around time),
for an additional fee of $60 per application.

Additionally, there is an option to Overnight a passport book once it is processed,
for an additional fee of $21.36 per application.
